Additionally, the lack of office-based systems for screening, the limited face-to-face office time with physicians, and the scarce community resources impact the ability to conduct SBIRT (Screening, Brief Intervention, and Referral to Treatment). <p> SBIRT is a public health approach where a comprehensive array of services is provided in a non-specialty health care setting. SBIRT is an evidence-based practice used to identify, reduce, and prevent problematic use, abuse, and dependence on alcohol and illicit drugs. The SBIRT model was incited by an Institute of Medicine recommendation that called for community-based screening for health risk behaviors, including substance use. It can be applied to a variety of behavioral health conditions, but has been found to be particularly effective in the reduction of risky alcohol use and promising in the decrease of drug consumption. <p> SBIRT is nationally considered one of the more successful models of integrated care. Since its inception, there have been over 100 studies that show SBIRT to be effective in multiple settings, including primary care. Due to its effectiveness and low cost, SBIRT has been recommended by the United States Preventive Services Task Force (USPSTF) since 2004. <p> This educational activity will give health care providers that serve in American Indian communities in North Carolina the skills needed to administer SBIRT for their patients. American Indians have high rates of substance abuse, so this training will be particularly beneficial to address this public health need. Specifically, we will review the prevalence and harms of alcohol and drug misuse, present the evidence base for SBIRT and its corresponding national recommendations, and introduce concepts of motivational interviewing and performance of brief intervention to address the knowledge gap. We will then practice performing brief interventions through role-play activities to improve physician competence in these areas, and provide examples of effective clinic systems to support SBIRT to help practice performance. Lastly, we will help identify those patients who cannot be treated adequately through brief intervention in the primary care setting, and need referral to specialty care. The attendees at our course will increase their competence on best practices in spasticity treatment and learn how to perform procedures involved in treatment including botulinum toxin injections through hands-on practice with cadavers and observation of live patient treatments. We will include a discussion of the problem of spasticity covering the current understanding of basic mechanisms and pathophysiology and their implications, review of the available forms of management and the pros and cons of each, consideration of non-pharmacological interventions and their role in overall management, e.g. physical therapy, etc. This will mirror the concept of managing the total patient from medications, therapy, and intervention if needed. Carotid duplex sonography, color-flow imaging, carotid intima-media thickness measurement, transcranial Doppler (TCD), embolus detection and new developments in ultrasonographic methods are presented during both lectures and hands-on scanning lab sessions. The emphasis is on clinical applications, case examples and principles of interpretation. The courses are designed as a one week curriculum, and participants are encouraged to attend for the entire week.
